Intermediate or after hospital care services are those provided to elderly people at home or in a residential setting for a short period of time. They help people with complex care needs to recuperate, recover, or convalesce, after an illness or hospital admission and to maximise their independence. The services aim to prevent unnecessary care home or hospital admission, and provide rehabilitation in a residential setting. This can help older people to regain or maximise their daily living skills, improve their confidence, and help them to remain in their home. Community rehabilitation of this nature also supports elderly people who are recovering from an illness through a period of convalescence, and it usually last for no more than 6 weeks.
